Payment is due at the time of service and is accepted in the form of cash, check, or credit card. Returned checks will be charged a $40 fee.
With the exception of PIP claims, Jessica does not provide insurance billing and will not be involved with any insurance related issues. However, she will happily provide you with a “superbill,” which you may submit to your insurance provider for reimbursement. Please contact your provider directly for their specific guidelines.
If you need to cancel or reschedule your appointment, please provide at least 24 hours notice to avoid a $50 late cancellation fee. Please note that missed appointments or “no-shows” will be charged the full cost of the visit.
